DESCRIPTION: Health and Homeless Systems Liaison will work at the systems level to improve the integration of and coordination between healthcare and homeless services in the Marion County and Polk County region. This will require strong, collaborative, and strategic leadershipThis position does not provide direct services.

ESSENTIAL FUNCTIONS/DUTIES:

  • Provide staff support to the Health & Safety Committee, including working with co-chairs to determine committee priorities in accordance with its purpose and core responsibilities, preparing agendas, taking minutes, and following up on action items.
    • The purpose of the Health & Safety Committee is to identify and address health and safety needs of people experiencing homelessness at a community/systems level throughout Marion County and Polk County and advise decision-making regarding:
      • Coordination between the Continuum of Care (CoC), healthcare entities, and housing/homeless service entities to strengthen partnerships;
      • Discharge planning for those currently homeless or at risk of homelessness from higher levels of care including correctional facilities, the youth foster care system, hospitals (including the Oregon State Hospital), and other physical health and mental health facilities;
      • Actively informing and utilizing the region’s Community Health Needs Assessment (CHNA or CHA) as it relates to homelessness and housing services;
      • Actively informing, utilizing, aligning efforts with, and supplementing the region’s Community Health Improvement Plan (CHIP or CHP) as it relates to homelessness and housing services; and
      • Emerging and/or pressing public health needs in alignment with the CoC’s Infectious Disease Prevention and Response Policy.
    • Manage the three-year Health, Housing and Homeless Service Initiative grant from the Willamette Health Council.
      • Oversee the implementation of all grant activities specified in the Letter of Agreement.
      • Prepare and submit biannual progress and financial reports.
      • Track spending against a budget of $1 million.
    • Lead projects that test and implement new strategies.
    • Identify system gaps and bottlenecks, provide education and/or propose new processes/ pathways to ensure coordination between systems, develop continuous improvement plans.
    • Plan, implement, and evaluate projects and/or pilots including defining scope, identifying support and available resources, developing timelines and workplans, and monitoring progress and documenting projects.
    • Create and manage contracts, incorporating measurable project outcomes, and monitoring compliance and deliverables.
    • Identify and analyze data to communicate impact and inform strategies for system improvements. Develop and deliver reports for funders and other key stakeholders as needed.
    • Serve as lead staff and active participant in the Marion-Polk Community Health Collaborative, participating in the Steering Committee and any other appropriate meeting groups, and connecting the MWVHA’s strategic plan and committee work to the Community Health Needs Assessment (CHNA or CHA) and Community Health Improvement Plan (CHIP or CHP).
    • Cultivate strong, collaborative relationships with healthcare and homeless service partners, and other key stakeholders.
    • Develop and track Key Performance Indicators (KPIs) to measure Health and Homeless Systems Liaison’s success.

ADDITIONAL RESPONSIBILITIES:

  • Draft scopes of work, process flows, and other documents as needed.
  • Serve as an advisor to MWVHA and local organizations on healthcare, housing, and homeless service integration best practices and methodologies.
  • Collaborate with the Board and other committees to communicate activities and progress of the Health & Safety Committee, Health, Housing, and Homeless Services Initiative, and other health related work.
  • Lead and participate in community forums regarding the intersection of homelessness and healthcare.
  • Other duties as assigned.

KNOWLEDGE/SKILLS/ABILITIES:

  • Must be a confident self-starter who can develop effective relationships, both internally and externally
  • Ability to navigate long standing systems with sometimes competing legal and contractual requirements
  • Strong listening skills and ability to find solutions with competing priorities and philosophies
  • Experience building cross-sector partnerships and collaboration
  • Proven ability to facilitate collaborative processes and drive consensus towards shared goals.
  • A critical and creative thinker, with a high level of initiative, motivation and enthusiasm
  • Knowledge of local homeless populations, systems and programs
  • Proven track record of strong, collaborative relationships with partners and providers
  • Local partnership experience highly preferred
  • Knowledge of local and state-level public systems of care including homelessness and healthcare
  • Knowledge of Oregon’s Medicaid/Coordinated Care Organization landscape, preferred
  • Knowledge of relevant research on the intersection of homelessness and healthcare preferred
  • Excellent organization, analytical, written and oral communication skills, including strong public presenting skills
  • Strong time management skills and the ability to multi-task and prioritize in a fast-paced environment, while exercising sound judgment
  • Demonstrates a sense of urgency, responsiveness, and attention to detail
  • Ability to prepare reports to track accomplishment of strategic goals and evaluate work performed
  • Proficient in Microsoft Outlook, Word, Excel, Zoom and other computer systems
